How to use this book report worksheet
The sheet walks a reader through a story in the order they experienced it. Students fill in the title, author and page count, circle or write how difficult the book felt, then retell the plot in three blocks — beginning, middle and end. Splitting the retelling into three keeps summaries from collapsing into “and then, and then, and then”.
It works as a reading-log entry, a homework page after a chapter book, or a quick comprehension check for a class novel. Because the prompts are generic it fits any fiction title, so one print run covers a whole shelf.
Printing notes
Both files are single-page vector PDFs — the text and rules stay sharp at any size, so scaling to A5 for a reading journal still prints cleanly. Print at 100% (not “fit to page”) to keep the writing lines at their intended spacing. Choose the US Letter file for North America and the A4 file everywhere else.
